Your First Visit

Kerhoulas Dental encourages you to complete our new patient form prior to your first appointment. If you are unable to do so, please arrive at least 10 minutes before your scheduled appointment to complete the form with our staff.*

Initial Examination

Your dentist will begin by thoroughly reviewing your dental and medical history, then performing a comprehensive dental exam. This will include an assessment of your teeth, jaw, bite and gums, an oral cancer screening and any X-rays that may be required. 

Treatment Plan

Based on your dentist's findings during your exam and your oral health history, your dentist will establish a long-term dental care plan for you. There will also be time to address any questions or concerns you might have regarding your dental health and treatment options.

• How can I book an appointment?

  Please contact our office during regular business hours to book an appointment at Kerhoulas Dental Rosedale.

• How do I cancel or reschedule an appointment?

  To cancel or reschedule an appointment, please call us. Please notify us of any change of plans as soon as you can, so another patient may be able to use the space in our schedule.

• Do you offer appointment reminders?

  You will receive automated, phone, text, or email appointment reminders in the days prior to your appointment. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.

• Do you provide services in any languages other than English?

  Yes, our office is able to provide dental services in several languages including Portuguese, Spanish, Serbian, Greek, Armenian, and Persian.

  If you have a language preference, please advise us when booking your appointment.

• Does your clinic have parking?

  Yes, we have paid parking available nearby. We are also accessible by public transit.

• Direct Billing to Insurance

  We offer direct billing for most insurance plans. For your convenience, we can submit your insurance claim electronically, resulting in faster reimbursement, without the need for paper forms and signatures.

  Our administrative team can assist you in understanding your insurance plan better and can also send estimates to your insurance company prior to starting any major treatments. 

  Please bring your insurance plan information to your appointment.

• Payment Methods

  Any co-pays or amounts not covered by insurance are your responsibility and must be paid at the time of your appointment.

  We accept payment by debit and most major credit cards.

*Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
